Schedule your annual Personal Health Assessment

Your Personal Health Assessment (PHA) includes a Biometric Screening and a Wellness Review.  The PHA gives you a snapshot of your current health status and potential health risks.  Your Wellness Review visit allows you to have a personalized analysis of your health and recommended steps for improvement.

GIFT: Receive a gift of appreciation upon completion of your Biometric Screening.  Gift incentives apply to employee, spouse, and any adult dependents.

SAVE: Receive savings on your health insurance premium upon completion of your Wellness Review.  Single employees will be credited a one month premium cost share. Married employees will be credited a one month premium cost share if both employee and spouse complete their Wellness Reviews. If only one of the married couple completes both steps of the PHA, the employee will save 50% of the monthly premium cost share. Savings incentives only apply to the employee and/or spouse. 

YOUR health: The PHA identifies risks for developing a preventable health condition. You will receive suggestions for personalized daily health habits and follow-up care, helping you to reach your health goals.

All employees, spouses, and adult dependents who are eligible for the Deschutes County Employee Benefits Plan can participate, including retirees and COIC employees.

Continuation of the PHA program is at the discretion of the Deschutes County Board of Commissioners.
